This is Mantra 1.2 of the Kena-upanishad (Kenopanishad), the English translation and commentary of Madhva (Madhvacharya) called the Bhasya. The Kena Upanishad deals with topics such as Brahman and Atman (soul) and also discusses the symbolic representation of the Gods as forces of nature. It is an important text in the Vedanta schools of Hindu philsophy. This is Mantra 2 of section 1 called ‘Prathama-Khanda’.

श्रोत्रस्य श्रोत्रं मनसो मनो यद् वाचो ह वाचं स उ प्राणस्य प्राणः ।
चक्षुषश्चक्षुरतिमुच्य धीराः प्रेत्यास्माल्लोकादमृता भवन्ति ॥ २ ॥

śrotrasya śrotraṃ manaso mano yad vāco ha vācaṃ sa u prāṇasya prāṇaḥ |
cakṣuṣaścakṣuratimucya dhīrāḥ pretyāsmāllokādamṛtā bhavanti || 2 ||

śrotrasya—of the ear; śrotram—ear; The transcendental cause, the giver of the power of bearing; manasaḥ—of the mind; manaḥ—the mind; The giver of the power of thinking; yat—because who; vācaḥ—of the speech; ha—verily; vācam—speech, the giver of the power of speaking; sa—He; u—indeed; prāṇasya—of the breath, or life Prāṇaḥ, life, leader, commander; cakṣuṣaḥ—of the eye; cakṣuḥ—eye; The giver of the power of seeing; atimucyaḥ—being free, having abandoned the “I-ness” in these; Freeing themselves from the bond of liṅga-śarīra; dhīrāḥ—the wise; pretya—on departing; Having renounced “I-ness” in all worldly objects; asmāt—from this; lokāt—world or body; amṛtaḥ—immortal, free, mukta; bhavanti—become.

2. Brahmā said, who is ear of the ear, the mind of the mind, verily He is the speech of the speech, He is the Praṇa of Prāṇa, and the eye of the eye, He verily (is Viṣṇu). The wise (knowing Him thus) after going forth from this world, and freeing themselves from the final body, become immortal.

He who is the ear of the ear (who gives to the ear the power of hearing), who is the mind of the mind (who gives to the mind the power of thinking), who is the speech of speech (who gives to the speech the power of speaking), is verily the Life of the First Life (who gives the power of life to the First Prāṇa), the ear of ear (who gives the ear the power of hearing). The wise, whose mind is under control, having known the Lord thus and realised Him, leave their final body through Suṣumna nāḍi and being released fully of their Prārabdha Karmas, become Immortal, i.e., attain mukti.
