The Chaitanya Bhagavata 3.5.741, English translation, including a commentary (Gaudiya-bhasya). This text is similair to the Caitanya-caritamrita and narrates the pastimes of Lord Caitanya, proclaimed to be the direct incarnation of Krishna (as Bhagavan) This is verse 741 of Antya-khanda chapter 5—“The Pastimes of Nityananda”.

সদাশিব-কবিরাজ—মহা-ভাগ্যবান্ যাঙ্র পুত্র—পুরুষোত্তম-দাস-নাম ॥ ৭৪১ ॥

सदाशिव-कविराज—महा-भाग्यवान् याङ्र पुत्र—पुरुषोत्तम-दास-नाम ॥ ७४१ ॥

sadāśiva-kavirāja—mahā-bhāgyavān yāṅra putra—puruṣottama-dāsa-nāma || 741 ||

sadasiva-kaviraja—maha-bhagyavan yanra putra—purusottama-dasa-nama (741)

English translation:

(741) Sadāśiva Kavirāja was most fortunate. He had a son named Puruṣottama dāsa.
