The Chaitanya Bhagavata 3.5.632, English translation, including a commentary (Gaudiya-bhasya). This text is similair to the Caitanya-caritamrita and narrates the pastimes of Lord Caitanya, proclaimed to be the direct incarnation of Krishna (as Bhagavan) This is verse 632 of Antya-khanda chapter 5—“The Pastimes of Nityananda”.

জন্মাবধি তুমি সে জীবের রাখ প্রাণ অন্তে ও তুমি সে প্রভু, কর পরিত্রাণ ॥ ৬৩২ ॥
जन्मावधि तुमि से जीवेर राख प्राण अन्ते ओ तुमि से प्रभु, कर परित्राण ॥ ६३२ ॥
janmāvadhi tumi se jīvera rākha prāṇa ante o tumi se prabhu, kara paritrāṇa || 632 ||

English translation:
(632) “O Lord, You protect the living entities from the time of their birth and also at the time of their death.
