The Chaitanya Bhagavata 3.5.478, English translation, including a commentary (Gaudiya-bhasya). This text is similair to the Caitanya-caritamrita and narrates the pastimes of Lord Caitanya, proclaimed to be the direct incarnation of Krishna (as Bhagavan) This is verse 478 of Antya-khanda chapter 5—“The Pastimes of Nityananda”.

“তুমি নিত্যানন্দ-মূর্তি নিত্যানন্দ-নাম মূর্তিমন্ত তুমি চৈতন্যের গুণ-ধাম ॥ ৪৭৮ ॥
“तुमि नित्यानन्द-मूर्ति नित्यानन्द-नाम मूर्तिमन्त तुमि चैतन्येर गुण-धाम ॥ ४७८ ॥
“tumi nityānanda-mūrti nityānanda-nāma mūrtimanta tumi caitanyera guṇa-dhāma || 478 ||
“tumi nityananda-murti nityananda-nama murtimanta tumi caitanyera guna-dhama (478)
English translation:
(478) “Your form is Nityānanda, and Your name is Nityānanda. You are the personification of Lord Caitanya’s transcendental qualities.
