The Chaitanya Bhagavata 3.5.452, English translation, including a commentary (Gaudiya-bhasya). This text is similair to the Caitanya-caritamrita and narrates the pastimes of Lord Caitanya, proclaimed to be the direct incarnation of Krishna (as Bhagavan) This is verse 452 of Antya-khanda chapter 5—“The Pastimes of Nityananda”.

জন্ম জন্ম নিত্যানন্দ-স্বরূপ ঈশ্বর জন্ম জন্ম উদ্ধারণো তাঙ্হার কিঙ্কর ॥ ৪৫২ ॥

जन्म जन्म नित्यानन्द-स्वरूप ईश्वर जन्म जन्म उद्धारणो ताङ्हार किङ्कर ॥ ४५२ ॥

janma janma nityānanda-svarūpa īśvara janma janma uddhāraṇo tāṅhāra kiṅkara || 452 ||

janma janma nityananda-svarupa isvara janma janma uddharano tanhara kinkara (452)

English translation:

(452) Birth after birth Nityānanda Svarūpa was his master, and birth after

Commentary: Gauḍīya-bhāṣya by Śrīla Bhaktisiddhānta Sarasvatī Ṭhākura:

birth Uddhāraṇa Datta was His servant.
