The Chaitanya Bhagavata 2.28.189, English translation, including a commentary (Gaudiya-bhasya). This text is similair to the Caitanya-caritamrita and narrates the pastimes of Lord Caitanya, proclaimed to be the direct incarnation of Krishna (as Bhagavan) This is verse 189 of Madhya-khanda chapter 28—“The Lord’s Pastime of Accepting Sannyasa”.

শ্রী-কৃষ্ণ-চৈতন্য নিত্যানন্দ দুই প্রভু এই বাঞ্ছা ইহা যেন না পাসরি কভু ॥ ১৮৯ ॥
श्री-कृष्ण-चैतन्य नित्यानन्द दुइ प्रभु एइ वाञ्छा इहा येन ना पासरि कभु ॥ १८९ ॥
śrī-kṛṣṇa-caitanya nityānanda dui prabhu ei vāñchā ihā yena nā pāsari kabhu || 189 ||
sri-krsna-caitanya nityananda dui prabhu ei vancha iha yena na pasari kabhu (189)
English translation:
(189) O Śrī Kṛṣṇa Caitanya and Nityānanda Prabhu, my only desire is that I should never forget You.
