It is a metaphor for profound and unforgettable gratitude (感激難忘 [gan ji nan wang]). From Liu Zongyuan (柳宗元 [liu zong yuan])'s "Memorial Expressing Gratitude for Being Relieved of the Post of Cishi of Liuzhou" (謝除柳州刺史表 [xie chu liu zhou ci shi biao]) in the Tang Dynasty (唐 [tang]): "Engraved in my heart and carved on my bones, there is no way to repay Heaven (無報上天 [wu bao shang tian])." Also written as "刻骨銘心 [ke gu ming xin]" (kègǔ míngxīn).
